One hundred and sixty-two patients operated on organs of lower part of abdominal cavity were studied. All the patients were divided into 3 groups depending on anesthesia method: general, epidural and spinal anesthesia. Efficacy of different methods of intrasurgical anesthesia for prevention of postoperative algetic syndrome and decrease of complications number was compared. It is demonstrated that spinal and epidural anesthesia ensure prophylaxis of postoperative algetic syndrome, but spinal anesthesia is the most simple, safe and cost-effective method.

[Surgical treatment of acute destructive pancreatitis with low frequency ultrasound]. / Khirurgicheskoe lechenie ostrogo destruktivnogo pankreatita s primeneniem nizkochastotnogo ul'trazvuka.

Energy of low-frequency ultrasound was used for removal of necrotic foci in the pancreas and parapancreatic tissue before septic complications. Studies of the pancreas of patients who had died of pancreonecrosis permitted to select optimum configuration of working butt of wave-wire. Experimental studies (22 dogs) demonstrated high efficacy of this method: necrotic foci were removed completely with little capillary bleeding; microscopic study showed that wound process had reparative character with minimum inflammation. Clinical study was carried out in 172 patients with acute destructive pancreatitis. In 139 patients traditional methods were used: left-sided resection of the pancreas, abdominization of the pancreas, staged sequestrectomies. Lethality in this group was 46.7%. Thirty-three patients underwent ultrasonic necrosequestrectomy, lethality was 30.3%.

[Gallstone ileus in cholecystoduodenal fistula]. / Zhelchnokamennaia neprokhodimost' pri kholetsitoduodenal'nom svishche.

Operations were performed on 15 patients with obturation gallstone ileus, 8 of them had a cholecystoduodenal fistula and the authors analysed their case records. It is most difficult to establish the diagnosis of the subacute form of the disease which is caused by slow movement of the stone in the intestine, which conceals the clinical picture and leads to delay of operation. The operation of choice is resection of the small intestine together with the stone or enterotomy with obligatory evacuation of contents from the proximal parts of the intestine. The mortality in the group of patients operated on later than on day 4 after the onset of the disease is very high and, according to our data, it is 100%. An obligatory condition of the operation is evacuation of the contents from the intestine in order to reduce intoxication and prevent endotoxic shock.

[Early and long-term results of the combined treatment of patients with gastric cancer submitted to cryosurgery]. / Blizhaishie i otdalennye rezul'taty kombinirovannogo lecheniia bol'nykh rakom zheludka s ispol'zovaniem kriokhirurgii.

The results of combined treatment of 28 patients with gastric cancer, using cryosurgery (cryodestruction and subtotal gastric resection), were analysed. In 36 patients of the control group, the subtotal resection alone was performed. In cryodestruction of a tumor, intraoperative dissemination of tumor cells significantly reduced, which was in negative correlation with the five-year survival of the patients. In the experimental group, the five-year survival was 80% in stage II, 47.8%--in stage III, and in the control group--40 and 22.6%, respectively.

[Ambulatory treatment of suppurative wounds with cooled glycerin]. / Obrabotka gnoinykh ran okhlazhdennym glitserinom v ambulatornykh usloviiakh.

Experiments were conducted on 106 albino rats under ether anesthesia. The effect of cooled glycerin, liquid nitrogen, ultrasonics, and their combinations on the course of an inflammatory process was studied. Cooled glycerin was used in the clinic in the treatment of 141 patients as the result of which the period of temporary incapacity reduced 1.4 times as compared to that in the traditional methods of wound cleansing.
